(We appreciate that there is a lot going on here over the past few days and we hope that you do not have email fatigue yet. We do try and limit the quantity of our emails. However, there are a number of deadlines coming up and it is important that you do not miss them).

You have until 11.59pm on Sunday 1st July 2018 to complete the 2018/19 Community Pharmacy Assurance Framework (CPAF) screening questionnaire.

We recommend you do this immediately. It doesn’t take long there are 10 questions you are required to answer and you are required to achieve a minimum of level 2. 

You can access the questionnaire using this link and then complete the questionnaire by adding in your F Code. 

The questionnaire has been revised for 2018/19, with two new questions as published in PSNC Briefing 032/18: CPAF screening questionnaire changes which explains these changes and provides clarification on some of the questions asked.
