Dear PharmaPlus Member,

The deadline to complete/submit pharmacy patient questionnaires for 2017/2018 is 31^st March 2018.

The members who have already ordered this via our nominated supplier Intellipharm, need to submit their completed questionnaires to Intellipharm latest by 2^nd March 2018 if they want to receive the final report by the end of March 2018 to meet the deadline.

For those of you who have not placed the order yet, we have attached an order form with this email.

Apart from this, pharmacy contractors also have to update their pharmacy practice leaflet by 31st July 2018 to include the new requirements. For more details see the link below from PSNC:

Practice leaflets can also be ordered from Intellipharm. Details and an order form is attached with this email.
