In this video we discuss the declaration process for the November 2017 quality payment review point. 

Remember the following:
  • You can declare from today if you have everything in place
  • You have until the 24th November 2017 to get all your quality payment materials in place
  • You have until the 8th December 2017 at the very latest to make the declaration
  • You must make your application to the RSPH today at the very latest as it can take 10 days to get registered for your Healthy Living Pharmacy

The documents that are mentioned in the videos can be downloaded here. Please ensure that you create your evidence folder in the following way if you have not done so already:
  1. Click here to access the file
  2. Then click on the 'Evidence Folder' and print out the material
  3. Place the materials in a folder
  4. Review action where necessary and print the remaining material and place into the folder
