Your Declaration
It is now time to Declare to the NHS!
We concentrate on how to declare for your payments. Remember that the review point is on the 28th April 2017 BUT you have until the 12th May 2017 to declare.

Remember to check through everything before you submit your declaration. You can not take it back once you send it! You MUST get it right the first time!

    The documents that are mentioned in the videos can be downloaded here. Please ensure that you create your evidence folder in the following way if you have not done so already:
    1. Click here to access the file
    2. Then click on the 'Evidence Folder' and print out the material
    3. Place the materials in a folder
    4. Review action where necessary and print the remaining material and place into the folder
