Next is:
STEP 10: Personalised asthma action plans (PAAPs) and promoting spacer device use in children prescribed pMDIs

What you need to do:

1.   Build the following into your daily routine for asthma patients.
2. All children aged 5 – 15 dispensed a press and breathe pMDI
      o   Check if they have a spacer device in line with NICE TA38
3. ALL patients aged 5 and over (including adults)
      o   Check if they have a personalised asthma action plan (PAAP)
4. If a patient does not have a spacer or PAAP they must be referred to an appropriate HCP (usually the GP practice).
5. Record your referrals on Pharmaoutcomes where a notification will be automatically sent to the patients GP practice (providing patient consent has been obtained).
6. Use the data collection form to keep a record of referrals for your declaration and post-payment verification checks.
7. If no patients have been identified for referral, you will still be eligible for payment if you can show evidence you have been working to identify suitable patients and have a process in place for identifying someone.
8. MYS declaration will require:
      o   Total number of patients referred for a spacer device.
      o   Total number of patients referred for a PAAP.
