The Hypertension Case Finding Service

Dear Member,

The hypertension case finding service is a new advanced service starting from 1st October 2021. The service will also be actively supported by PCNs, who must improve the diagnosis of patients with hypertension.

The Service

The service is set up into two parts. The pharmacy MUST offer both elements of the service, and cannot just offer only one part.

1. Identifying patients and offering a blood pressure check.
2. Where it is clinically indicated, the pharmacy then will offer a 24 hour ambulatory blood pressure monitoring service and then inform the GP of the result.

2.   Download and read the service specification & the PSNC briefing on the service.
3.   Read the NICE guideline Hypertension in adults: diagnosis and management.
4.   Complete the CPPE hypertension gateway page (optional).
5.   Training on use of the blood pressure monitors (individual to each monitor).
6.   We will be publishing our SOP shortly.

What you need (both machines are required):

1. A ‘normal’ BP monitor. A list of the monitors you can use can be found here: Validated BP Monitors for Home Use
2. An ABPM for 24 hour monitoring. A list of these can be found here: Validated BP Monitors for Specialist Use

PharmaPlus is currently sourcing machines that you could potentially buy or rent. We will inform you shortly of the outcomes.

You should also be aware that BP machines, in particular ABPMs, require routine calibration and training. We are looking into how best this can be delivered to you as each machine is different. It also requires the use of computer software and insurance arrangements because of the cost of the machines.

The cost of the ABPMs can be several hundred pounds, with additional costs relating to insurance and calibration.

What you need to do once set up:

1. Register to provide the service via MYS
2. Update your NHS Website with the new service

Funding and payments if you start immediately.

Set-up fee (once you have signed up via MYS)	£440
BP check	£15
ABPM check	£45
If you do 5 ABPM checks in 2021/22	£1000
If you do 15 ABPM checks in 2022/23	£400
If you do 20 ABPM checks in 2023/24	£400
