You must submit your declaration for the Pharmacy Quality Scheme (PQS) by 11.59pm on 28^th February 2020.

Here is what you need to do:

1. View our checklist attached to ensure you have met all the criteria & logon to the PharmaPlus website to view all our resources  
2. Click here to view our video on the claiming process
3. Check you have met gateway criteria using the PQS Dateset
4. All declaration questions can be downloaded by clicking here
5. Logon to MYS to make your declaration
   1. You must meet all the quality criteria in each domain to be eligible for a PQS payment.
   2. The PQS payment will depend on how many of the domains the pharmacy meets.
6. Only submit your claim once you are sure it has all been accurately completed – This cannot be amended once submitted
7. Keep your email confirmation safe – it may go into your Junk Folder.
