As part of the risk management and safety domain category of the Pharmacy Quality Scheme you are required to have a new written patient safety report available for inspection at your premises.
This category is worth between £1920 - £3840 provided all criteria are met. This includes CPPE risk management and assessment, CPPE sepsis online training and assessment, risk review, CPPE LASA e-learning and assessment and a written patient safety report.
What you need to do to meet this criteria:
- Have a new written patient safety report (i.e. since 15th Feb 2019) or covering the last year
- Collect your monthly patient safety logs from March 2019 to date – Click here to download the template

- Things to include in the report
- Analysis of incidents and incident patterns
- Evidence of sharing locally and nationally
- Actions taken in response to national patient safety alerts
- LASA learnings – From CPPE, review, actions, etc
- Actions taken to prevent risk
You must be able to demonstrate that actions have been put in place to prevent these risks, for example, physical separation, staff awareness raising, visual warnings, tags or labels on shelving, fatigue reduction strategies or enhanced checking procedures for these medicines.

• Carbamazepine and carbimazole;
• Rivaroxaban and rosuvastatin; and
• Atenolol and allopurinol
