Dear member,

With regards to the PQS - Dementia Friends and Dementia Friendly Environment, below is what you need to do to achieve this criterion:

PharmaPlus Calendar 2019

Please find attached a 2019 PharmaPlus Community Pharmacy Calendar 2019 containing information on key deadlines. We will continue to add to this calendar as more information is released.

How to become a Dementia Friend

ALL patient facing staff are to become dementia friends (includes your delivery drivers & locums)
Complete this via the Dementia Friends Pharmacy Portal
Register & watch the introductory video PLUS at least ONE other video
Order your dementia friends badges by emailing - programmepartnerships@alzheimer.org.uk with the registration code and total number of staff who have become dementia friends & address of the pharmacy

Dementia Friendly Environment Checklist

You will need to assess your pharmacy environment to ensure that reasonable steps have been taken to support the needs of people affected by dementia
Complete the dementia friendly environment checklist attached
Create an action plan which includes making some demonstrable recorded changes to the environment
Some actions identified may need to be considered as part of future development of the pharmacy premises
