Dear Member,

As part of the Risk Management & Safety Pharmacy Quality Scheme (PQS) Bundle you will need to complete the following Risk Management Activities:

1. CPPE Risk Management Training & Assessment
   1. To be completed if you did NOT complete it for 2018/19 QP
   2. To be completed by 80% of registered pharmacy professionals
2. Risk Review – See attached template
   1. If you completed a risk review for 2018/19 QP then you are only required to update your previous risk review
      1. Recorded reflection on indentified risk and the risk minimisation actions
      2. Any subsequent actions identified as a result of reflection
   2. Risk of sepsis
      1. Risk of missing sepsis identification as a new risk
      2. Risk minimisation actions undertaken to mitigate the risk
      3. Completion of Sepsis training
   3. If you did NOT claim the risk management criterion previously you must undertake a risk review
      1. Identify 2 risks including the risk of missing sepsis as above
