- Stoptober starts 1st Oct 19 till 31st Oct 19 – Change materials to Stoptober
- Complete the campaign audit form for AMR (These vary locally - London pharmacies see attached)
- 1st Nov 19 complete campaign audit form for Stoptober
- Keep these audit forms on record in your public health folders in case of an CPAF inspection or request from NHS England
- Check you have carried out audits for all the health campaigns this year so far (you can adapt the attached audit titles if you have not)
We have attached the campaign audit forms for AMR & Stoptober – please note these are forms being used for London pharmacies. You local area may have a different form being used. If you have not been issued a specific audit form from your area team contact your area team or you may use the audit forms attached.
