Complete Your CPAF Questionnaire

Dear Member,

CPAF is a Terms of Service requirement. Completing the screening significantly reduces the likelihood of being selected for the full CPAF questionnaire.

Questionnaire details:
    •    10 questions
    •    Takes 10–20 minutes
    •    Complete via MYS portal or online (if not on MYS)
    •    Questions are on the NHSBSA website

If you’ve not received the questionnaire or can’t access it, email: pharmacysupport@nhsbsa.nhs.uk

Action Plan:
    •    Check you’ve received the CPAF questionnaire
    •    Complete it by Thursday 31 July 2025
    •    Contact NHSBSA or PharmaPlus if needed
