We've linked the symptom checker checklists and accompanying documents for each of the seven conditions for the Pharmacy First pathway. The checklists should help you to distinguish whether a patient would be eligible for the service and, if not, what the best course of action would be according to their answers.

The checklist is for the patient to fill out themselves.

You should laminate the checklist so it can be used over again to save paper. The patient can use a whiteboard pen to answer the questions.
