High Risk Medicines - Anticoagulation Audit

This year the medicine safety and optimisation domain requires us to complete an anticoagulation audit.

Anticoagulants are high risk drugs with a potential to cause significant harm. Pharmacy teams can play a significant role in preventing harm by providing patients or carers where appropriate with information and support.

What Do You Need To Do

• Over a 2-week period this month identify 15 patients on anti-coagulants and complete the following form for each patient.

• Any patient referred to their GP, as well as details of what action was taken should be followed up before declaration date (31^stMarch 2023)
• If within this period you have not been able to identify 15 patients you are able to extend this audit by another 2 weeks.

How to claim and other considerations

Declarations to be made on the MYS portal that by 31^st March 2024 that you will have completed the anticoagulant audit.

Don’t forget to note the start and end date as you will need this for your MYS declaration.

A declaration that where concerns are identified when completing the audit, that the patient’s GP was/will be promptly notified.

If the required number of patients have not been identified, then a declaration that no patients have been identified as being suitable for audit.

IMPORTANT: No patient identifiable data should be entered into the MYS portal.
