2. TARGET Treating Your Infection Leaflets
What to do
When - Best done before the Feb claim date- (5th February 2024 and 1st March 2024)
2.1) Within a
4-week period appropriately identify 15 patients who would benefit from discussion around
a) Treating your infection - UTI (leaflet attached)
b) Treating your infection - Respiratory (leaflet attached)
2.2) If 15 patients for each have not been identified within this time frame, then this can be extended by another 4 weeks (totalling an 8-week period to complete this)
2.3) Record the start and the end date for claiming.
How to claim on MYS
- A declaration
that by the end of 31 March 2024 (ideally before the claim window) the TARGET Treating your infection leaflets will be completed.
- Report the start and end date of the review.
- A declaration that the prescriber was contacted when concerns were identified (if any)
- A declaration that 15 patients were identified for each ailment.
- If applicable declare that no patients have been identified as being suitable for review, but demonstrate that attempts were made to identify appropriate patients, i.e. through your SOP, action plan or training.
