The CPAF screening questionnaire has been available since 18th July 2022. The deadline for completing this is TOMORROW 31st August 2022.
You are required to complete this form through the MYS portal under the NHS Terms of Service. If you have not received an email from the NHSBSA before this time or have any queries on it, then you will either need to contact us for further help and guidance or email nhsbsa.cpaf@nhs.net

1) Minor changes from last year. The section on SOPs has been removed, and a new area for opening hours is included.
2) Answer the questions as truthfully as possible, and bear in mind that the NHS may require you to provide evidence if needed
3) There are three levels. A minimum level of 1 and 2 needs to be attained for each question. Level 3 is what you should be aspiring to.
