Dear Member,

You should have received your weight management campaign materials by now – this is a mandatory health campaign and therefore you MUST participate.

What you need to do:

1. Display the campaign materials and download any digital resources required from the Campaign Resource Centre website.

2. Promote and run the campaign for 5 weeks – Record the dates you have run the campaign for.

3.   PQS – Weight management – Must be completed by 31st March 2022 - If you have yet to complete this we recommend you run both of these alongside each other.
  a.   Please see the PharmaPlus PQS section of the website for full details and resources on how to complete your PQS tasks.
  b.   Download our intervention & referral guide (see attached).
  c.   Must have access to equipment to accurately measure height, weight and waist circumference.
  d.   Intervention Stage (minimum of 25 patients needed).
  e.   Referral stage (minimum 1 referral).
  f.   Must undertake the minimum patient numbers for both stages to qualify for full points.
  g.   Use the data collection form to record your interventions and referrals.

4. Understand the NHS Digital Weight management programme and how to refer.

If you have not received your campaign materials – email partnerships@phe.gov.uk with ‘Weight Management Pharmacy Packs’ in the subject line and include your pharmacy name and address
