1. Submit your end of month FP34C via MYS
a. Print off the populated FP34C from MYS
b. Sign, stamp and date
c. Include in your bundle of paper prescriptions
2. You will receive your advanced payments 20 days earlier if you submit via MYS by the 5th of the month.
3. You will receive 100% of your advanced payment 4 working days after submission deadline of the 5th.
4. Paper forms will stopped being issued from March 2022 therefore we encourage you to make the change now!
5. Paper FP34C submissions will NOT receive any earlier advance payments but instead will receive advance payments in accordance with the current payment timetable.
6. Late administrative charge of £25 for late submissions after the 5th of the month from April 2022 and no early advanced payment will be made.
